Foster Home Questionnaire
Instructions for submitting this form either ON-LINE, by FAX, or by MAIL are at the end of the form......

A foster home is a temporary home for one of our available Greyhounds. By being a foster home you allow room for a Greyhound to be brought in from the breeders/owners/trainers and assist in getting the Greyhound acclimated to home-life and prepared for adoption. Most of our foster homes will have their foster Greyhound for a 2 week minimum, depending on the availability of an appropriate prospective applicant "match". By submitting this questionnaire, we can better "match" a foster Greyhound to your particular home environment! Thank you!!

Tell us about you!

Your name (include spouse if applicable):
Your street address: City:
State: Zip Code:
Your email address:
Home Phone: Work Phone:

Tell us about your pets!

Where will your Foster Greyhound stay during the day?
Do you realize that the Foster Greyhound you receive may never of been in a home before and you will be responsible for the care, training, house-breaking, and the gentle teaching of 'general' good house manners?
Do you have cats?
Do you currently own other dogs? How many?
Would you be willing to attempt to use our 'in-house' Vet located in St. Pete, FL who gives us a substantial discount on his services?
Are your pets current on vaccinations?

Accommodations for your Foster Greyhound:

Type of your dwelling:
Do you have a fenced in yard?
If you do not have a fenced in yard, are you willing and able to walk your Foster Greyhound on a leash 3 to 5 times a day to relieve itself? (Possibly more in the beginning to facilitate house-breaking?)
Do you have a swimming pool, lake, or pond in or near your yard (accessible by the Foster Greyhound)?
Do you understand the importance of keeping your Foster Greyhound on a leash at all times when it is outdoors in an unfenced area, and are you willing to always abide by that condition?
Do you agree to keep a collar bearing identification (the GCGA tag) on your Foster Greyhound at all times?

Other things involved in fostering a Greyhound:

1. Are you willing to bring your foster Greyhound to the scheduled Meet and Greets (within a reasonable distance from your home)?
2. Are you willing to transport your foster Greyhound to and from Meet and Greets even if you can't stay?
3. Are you able to show your foster Greyhound to prospective adopters at your home with the assistance of an GCGA Placement Rep?
4. Do you have a crate for your foster Greyhound? (We have several available for that purpose if not).
5. Do you understand the importance of crating your foster Greyhound in the beginning? Are you willing to crate your foster Greyhound when you are unable to supervise their interactions with your pets and your home?

By submitting this questionnaire you are greatly facilitating our fostering process and are helping us to correctly match a foster Greyhound to your home. Thank You for considering fostering a Greyhound for GCGA!!
